The Care Coordination team at Nice Healthcare has an exciting opportunity for an experienced Registered Nurse (RN) Care Coordinator.

We are looking for a motivated, resourceful and organized individual to provide amazing services through patient care coordination and Provider support. In this role, the RN Care Coordinator will work independently providing professional appropriate telehealth nursing services to patients and/or their family members.This is done by utilizing a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act and its recognized professional standards. You’ll work collaboratively with the clinical team to help support our Physician Assistants and Nurse Practitioners to provide the best healthcare for our patients.

What you’ll do at Nice:

RN Care Coordinator

  • Utilize the Nice app, patient electronic medical record, and chat platforms to communicate with patients and staff
  • Respond to incoming messages from patients and perform advanced triage communication between patients and clinical staff in a timely manner. The advanced triage protocol/process will allow the triage professional to assess the patient and intervene with diagnostic and therapeutic approaches by adhering to the core principles of the treatment plan even before the patient is referred back to the provider
  • Will function as a liaison between providers, patients, and staff in the delivery of complex virtual care
  • The nurse must be a patient advocate, maintaining the patient’s confidentiality and patient rights
  • A RN care coordinator will also spend time educating patients about their conditions, connecting them with health care providers, and evaluating their progress.
  • The RN will resolve patient concerns or complaints while maintaining a positive, personalized and friendly attitude, addressing patient needs in a timely and appropriate manner
  • Initiate patient referrals via electronic fax submission to speciality clinics
  • Triage lab and imaging results, and notify providers of abnormal findings or critical values
  • Perform medication refills and prior authorizations
  • Troubleshoot and track pharmacy related errors
  • Occasionally deliver lab results and patient education via the Nice app
  • Provide support and coverage for other members of the Care Coordination team
  • Ensure every communication and interaction between Nice and its patients is a positive and professional experience
  • Functions as a mentor to other team members and works collaboratively with team members to identify and resolve department and/or guideline issues through established procedures
  • Supports a team environment which allows clear communication, positive support and creative thinking

What you’ve done before:

  • Have an active, unrestricted RN License in respective state(s) and/or hold a compact license
  • Ability to obtain additional state licensure as needed
  • Prior triage and clinical practice experience in ambulatory, virtual or telephone triage highly preferred
  • Strong attention to detail, prioritization, organization and self discipline
  • Critical thinking skills, such as ability to work through processes that are multifaceted
  • Extremely effective communication with team members using various web-based interoffice communication platforms
  • Able to work efficiently in a virtual team setting while working independently from home
  • Ability to efficiently transition between multiple digital platforms
  • Experience with technology including, but not limited to web-based communication, virtual mapping platforms, virtual teleconferencing software, online password managers, software-based authenticators, MS Office and Google Suite

Position Specifics:

Job Type: Full Time

Hours:

  • Monday-Friday (8 hour shifts)
  • Weekends: Every 4th weekend (4 hour shifts)

Location:

  • Fully remote

Job requirements:

  • Strong Internet signal with a secure wifi
  • A dedicated work space/office, free from interruptions is required
  • 3-5 years experience as a Registered Nurse in an ambulatory setting
  • At least one year experience as a Registered Nurse in a virtual or ambulatory primary care setting (strongly preferred)
